The Primary Care Division, within Public Health Wales, are looking for a skilled, experienced, and motivated public health specialist, to support the monitoring, evaluation and transformation of an integrated primary care and public health system in Wales.

The post holder will work closely with partners and colleagues to bring a rigorous evidence-based approach to monitoring and evaluation of the primary and community care transformation programme.

The post holder will lead monitoring and evaluation activities within the Primary Care Division and will work with a wide range of partners and colleagues, to conduct, commission and develop evaluation skills.

Partners include the national Strategic Programme for Primary Care (SPPC), Welsh Government, other central and local public health teams, primary care professionals, primary care clusters, professional collaboratives and colleagues in health boards or other organisations who support them directly.

This partnership approach will require excellent communication skills and an understanding of primary and community care as a setting for public health action. Combined with the knowledge and expertise of effective strategies for monitoring and evaluation and improving health in this setting.

**The main duties will include**:
Advise on aspects of monitoring and evaluation across key Strategic Programme for Primary Care (SPPC) and Primary Care Division workstreams, programmes and projects.

Provide expert advice on monitoring and evaluation requirements for implementing the Primary Care Model for Wales (PCMW) and Accelerated Cluster Development (ACD) programme.

Take a strategic approach and work across boundaries to strengthen primary care public health monitoring and evaluation in Wales.

Work with other divisions to take forward the implementation of the Public Health Wales Research and Evaluation Strategy and Quality and Improvement Strategy in relation to the Primary Care Division’s work programme.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

This is a unique opportunity for a skilled, experienced and dynamic individual to join the Primary Care Division in Public Health Wales as our Public Health Monitoring and Evaluation Lead for Primary Care. Working at a national level under the direction of a Consultant in Public Health/ Public Health Medicine and as part of a multi-skilled team, the post holder will support the transformation of primary and community care, across Wales. Working within the legislative and policy context of the world-leading Wellbeing of Future Generations (Wales) Act and A Healthier Wales strategic vision.

The post holder will need to show evidence of a successful track record of working in a role primarily focussed on monitoring, evaluation and / or research. Demonstrating achievements and experience of practice at a senior level.

The post holder will require a good knowledge of the legislative and policy context in Wales, including developments within the primary and community care setting and the wider Health and Social Care system in Wales.

A high level of intellectual rigour, negotiation and motivation skills, flexibility and management skills are required to deal with complex policy, research, and evaluation (including return on investment methodologies/ applied health economics), and advocacy issues. In addition, an understanding of other cultures and systems of work, to enable effective working across organisational boundaries to influence transformation and large scale change.
