**Position Overview**:
As a Healthcare Assistant/Support Worker at **Innovae Care**, you will play a vital role in providing essential care and support to our clients. You will work closely with our team to ensure the well-being and comfort of clients, making a significant difference in their lives. **Responsibilities**:

- Provide direct care and support to individuals in need, including those in assisted living, hospice care, or with behavioral management needs.
- Assist with case management by documenting and implementing care plans for each client.
- Support clients with daily activities such as meal preparation, personal hygiene, and medication administration.
- Ensure the safety and well-being of clients by monitoring their condition and responding to any changes or emergencies.
- Collaborate with healthcare professionals to provide comprehensive client care.
- Maintain accurate and up-to-date records and documentation.

**Skills**:

- Strong communication skills to effectively interact with clients, their families, and healthcare professionals.
- Ability to manage challenging behaviors and provide behavioral management support.
- Experience in assisting individuals with activities of daily living (ADLs).
- Ability to assist clients with mobility needs.
- Please note that a valid driver's license may be required for this position.
