**PURPOSE OF THE POST**
- To develop and deliver the Social Provider Service (SPS) in partnership with GP surgeries which will enable adults to improve their health and wellbeing and reduce their demand on statutory health services. The focus in SP is on non-medical needs affecting wellbeing i.e., psycho-social-economic needs of the patient.
- To lead and support Social Prescriber Volunteers.

**Duties and Responsibilities**
- Receive referrals from GP practices and arrange to contact patients, assess their needs, identify goals and appropriate action for support, and draw up a flexible person-centred plan. When appropriate to the case allocate patients to volunteers.
- Offer patients direct support and/or refer to appropriate agencies, including Adult Social Care or primary care services.
- Assist patients to access community care assessments and medical appointments.
- Identify when there is a need for urgent action or for a step-up in care and communicate with the relevant professionals.
- Maintain database records of interventions with patients and outcomes of referrals/signposting and communicate as required with health professionals.
- Conclude action plans after a defined number of episodes of contact. Evaluate and record client outcomes. Provide regular reports on activity and outcomes and case studies as required.
- Ensure carers are identified and supported as appropriate, including signposting for carers’ assessments.
- Develop comprehensive knowledge of local available services from the community, voluntary and statutory sectors.
- Support Social Prescriber Volunteers to work within the team, mainly case management and client matching.
- Market the service so we receive the referrals needed.
- Work collaboratively with other social prescribers and manager to ensure the sharing of best practice and the covering of the service during times of staff absence.
- Foster positive relationships with GP practice staff and other medical and community healthcare professionals.
- Attend team meetings, induction events, training, staff conferences etc and be aware of AUKES services and products.

**Quality processes**
- Comply with the AUKES’s quality standard ISO 9001 and other quality standards such as Investing in Volunteers, as necessary.
- Ensure that the service templates (standard documents) are used for administration.
- Undertake regular customer satisfaction surveys and provide monitoring information as required by the Team Leader.

**OTHER**:

- To work within the policies, procedures and quality frameworks adopted by Age UK East Sussex with particular attention to equal opportunities, data protection/confidentiality, safeguarding and health and safety requirements.
- To undertake relevant training as required by the organisation and agreed with your line manager.
- To be an Ambassador for the Charity, positively promoting the organisation and its services at all times.
- To undertake other such responsibilities and duties as may be reasonably required by your line manager within the level and grading of the post and to work flexibly as required.

**Essential Requirements**
- Minimum of 5 GCSE Grade C
- Relevant additional training in the health and Social care field, including safeguarding, motivational interviewing, mental health awareness and older people.
- A minimum of 2 years active and successful experience of working with people (ideally older) within a health/ social care or information and advice setting
- Experience of person-centred care planning
- Knowledge and understanding of issues which can impact on the health and wellbeing of older people.
- Ability to work unsupervised, set efficient goals and timeframes and good at multi-tasking
- Excellent interpersonal and communication skills (including the ability to write clear reports and communicate sensitively with people at all levels).
- Confidence to market the service within GP surgeries
- Ability to work co-operatively and effectively as part of a team.
- A professional and compassionate outlook
- Understanding of the principles of equality and diversity.
- A capacity to be focused and clear on boundaries
- Full driving licence, access to a vehicle and willingness to travel throughout East Sussex (for which mileage will be paid).
