**Role**:
To provide a comprehensive service to a caseload of clients who are currently sleeping rough in Basingstoke & Deane. Building trust and providing person-centered advocacy and support, which enables clients to navigate pathways and progress towards sustaining a life away from the streets and into sustained accomoodation.

Note: This will include working weekends and unsocial hours and being able to walk a number of miles in all weather conditions

**Responsibilities include**:

- To undertake assertive outreach and hot spot visits, including during unsocial hours, to engage with people sleeping rough
- Carry out a strength-based assessment with each client and plan and enact support actions / interventions
- To recognise indications of substance misuse, mental health needs, domestic abuse, trauma-based behavioural issues and appropriate referrals to relevant agencies
- Support close partnership working with the housing, health and wellbeing services, police, emergency, accommodation providers and wider partner agencies to maximise positive outcomes for clients
- Since job descriptions cannot be exhaustive; the post holder may be required to undertake other duties, which are broadly in line with the above key responsibilities

**Qualifications / Requirements**:

- Experience working with vulnerable adults and/or people with complex needs
- The ability to maintain professional boundaries with clients and assess risk effectively

**Job Types**: Full-time, Permanent

**Salary**: £22,440.00-£23,812.00 per year
