An exciting opportunity has arisen for a full time office based Band 4 Falls Prevention Coordinator to join our busy, well-established Falls Prevention Teams in Worthing and Chichester which covers the West locality of Sussex Community NHS Foundation Trust.

The ability to work autonomously with excellent communication skills is essential as the post-holder will need to liaise closely with members of the multi-disciplinary team including Nurses, GPs and other Health Care Professionals.

Computer literacy and proficiency in Microsoft word, Excel, Outlook & NHS mail is essential with a flexible attitude. They will be a strong team player with excellent communication skills.

This is a demanding but enjoyable role, which will give the successful applicant the opportunity to develop their career in a varied post, liaising with primary and secondary care.

The Falls coordinator will take referrals for falls services and ensure that all referrals are processed following local policies and pathways and accurate signposting is offered to those whose queries cannot be dealt with over the telephone.

To maintain the information systems used by the team in order to collate and co-ordinate performance data and produce reports as required for the team and relevant managers.

This post will form part of a dynamic team delivering specialist assessment and tailored rehabilitation programmes to those who have, or are at risk of falling in the well established Worthing and Chichester Teams.

The Falls coordinator will receive all falls prevention referrals, messages and queries by letter, phone, fax, electronically and face-to-face as the Single Point of Access for Falls and use local pathways and protocols including the Falls Risk Assessment Tool (FRAT) to process these referrals in a timely manner to the appropriate teams.

To liaise with referrers to request further information as appropriate, such as patient summaries from GPs

To be familiar with exclusion/inclusion criteria for falls services and return referrals that are inappropriate, seeking advice from falls clinicians when unsure and signposting where appropriate.

To assist the team with other administrative tasks related to the Falls Prevention Team.

To contact low risk falls patients by telephone to offer a referral to community Wellbeing services, home exercise leaflet and other options as they are developed.
