**ABOUT THE ROLE**:
Everyone we support are referred via adult social care teams or other professionals within hospitals or I.C.B. Everyone we support must qualify for health or social support funded by adult social services or I.C.B. The purpose of this role will be to respond to social workers, commissioners or other professionals when they enquire about our services and progress then through our eligibility criteria.
We will then arrange an assessment of the individuals support needs and show them what we offer and how we can support them. The role is to build and maintain positive relationships with local authorities and ensure there is clear written assessments and plans to meet the person needs. Working closely with the Director of services to ensure we match people appropriately with the right people and homes.
**Main duties of the Referrals Coordinator include**:
- To take and make any phone calls regarding referrals and assessments.
- To ensure that our waiting list system is kept organised and up-to-date
- To carry out initial phone calls with those referred before arranging a face to face assessment
- To carry out face to face assessments with those referred to our service
- To communicate with the team and keep the relevant team members up-to-date and information share

**Required skills/experience**:
- Experience of working with those with multiple complex needs
- Effective communication skills, both written and verbal

- Great organisational skills
- Experience in carrying out assessments of people
- An understanding of Supported Accommodation and or homelessness
- Experience in using Microsoft packages
- Computer literate
- Effective decision making skills
**Desired skills/experience**:
- Experience in liaising and working in partnership with other organisations.
- Experience in assessing those who have a learning disability, autism, mental health and behaviours that challenge.
**LOCATION**:
This is an office based position, our head office is located just off the bottom of Carlton road Nottingham, NG3 2EG.
**HOURS**:
This role is OPEN TO FULL TIME OR PART TIME hours. Monday and Friday,
