**Job Title:** Substance Misuse Treatment Coordinator

HMP Nottingham, part of Nottinghamshire Healthcare NHS Foundation Trust, seeks a skilled Substance Misuse Treatment Coordinator to join their team.

As a key member of the Substance Misuse Team, you will be responsible for coordinating and delivering evidence-based treatments to patients struggling with substance misuse.

Your primary focus will be on providing high-quality patient care, working collaboratively with multidisciplinary teams to ensure seamless care coordination and timely interventions.

**Key Responsibilities:

  • Coordinate and deliver substance misuse treatment programs, incorporating evidence-based interventions and tailored care plans.
  • Work closely with multidisciplinary teams to ensure timely referrals, assessments, and interventions.

**Requirements:

  • RGN/RMN Registered Nurse with experience in addiction services and mental health
  • Demonstrated expertise in program coordination, case management, and clinical documentation
