Our programmes are designed to support individuals and families to improve their health behaviours and make sustainable changes to their lifestyle. The IHWS programme in Nottingham consists of five key services;
- Stop smoking support
- Tailored physical activity programmes
- Adult weight management
- Child/family weight management
- Health Coaching

Residents across the City of Nottingham will be able to benefit from a variety of our established pathways and products, including Gloji, Slimming World and MAN v FAT Football. As a Primary Care Coordinator, you will be responsible for building successful working partnerships with colleagues across primary and secondary care in order to promote the programme and maximise referrals.

A typical day as a** Primary Care Coordinator**_ _might include:

- Forming, developing and maintaining relationships within the Primary Care Network (PCN)
- Attending networking/outreach events and PCN board meetings to share information about the programme, ensuring colleagues in primary and secondary care are aware of the eligibility criteria and referral routes
- Engage in joint up working with primary and secondary care to develop effective and efficient referral routes in to the programme
- Using population health data to identify key priority groups in order to focus referral generation activities
- Generating and sharing KPI overview referral and outcome data with the PCN and secondary care settings
- Working together with primary care colleagues to create targeted messages/letter to patients, whilst supporting their population health goals
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments - we’re always looking to share knowledge and resources
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values

To work with us in this role you will need to have some knowledge or experience in networking, referral generation and/or partnership management, as well as an understanding of health inequalities and approaches on how to engage those experiencing them into health promotion services. It is also important that you are able to navigate the primary care landscape, with an understanding of how patients access care - bonus points if you have worked in a primary care setting previously
