**Summary of Role**

The Care coordinator will report to the Practice Managers and work closely with the management team at Park Surgery. This role will be dedicated to improving outcomes for The Park Surgery patients and coordinating their care, increase uptake and management of locally commissioned services (LCS), Quality and Outcomes frameworks (QOF), Prescribing incentive schemes (PQS) and the Impact Investment Fund Initiatives (IIF). They will support the management team with the smooth running of the surgery and work closely with reception leads to identify and resolve any clinical staffing issues.

You will work alongside the administrative & reception teams to learn their roles and how you can utilise their skills to support the practice maintain and achieve the annual targets we are set. You will work closely with the practice management team around reporting, filing and other HR duties.

You will be part of the surgery team and support the development of strong team spirt, commitment and enthusiasm with all staff.

You will attend practice meetings keep updated and have a good knowledge of NHS primary care.

You will also need to work closely with other external parties such as PCN staff to continuously develop and improve the services we offer.

You will at times be the point of contact for patients and outside agencies and as a representative of the surgery will be required to be courteous, sympathetic and professional at all times.

**Overall purpose of the role**

You will assist with the daily management of the practice and will ensure staff are supported. You will address issues as they arise to keep the service running smoothly. You will also work closely with the Practice Managers to develop the financial performance and productivity of the practice.

**Main Duties and responsibilities**
- Supporting the Practice Managers as required with projects, systems and procedures
- Act as a point of contact for everything QOF, IIF, LCS, DES related
- Undertaking tasks as directed by the Practice Managers in areas of change management and continuous improvement
- Liaise and support the Reception Lead together with the administration team, and at times the nursing team, to ensure the smooth running of the practice.
- To manage and support the computerised appointment system, to ensure that all appointments for all the clinicians are entered onto the appointments system a minimum of one month in advance.
- To manage the diary for all clinicians’ holidays, meetings, etc and to make sure all the information is incorporated into the appointment schedules.
- Responsibility for data quality, ensuring read codes are correctly administered and understood, providing advice, support and training for all staff as and when required.
- Assist the Practice Managers with various claims process to provide maximum income for the practice.
- Explore ways of enhancing practice performance in key areas.
- Analyse and maintain performance in the following areas Quality and Outcomes Framework/Access to services/Enhanced Services/Incentive Schemes/Patient and public involvement/PPG/Information Governance.
- To formulate new policies and update existing ones as deemed necessary and set up systems for their implementation and maintain policy register.
- To ensure that patients are receiving the best possible service and patient services are regularly reviewed
- To support with the provision, gathering and or investigation of information and data for Performance Reviews, Audits, Reports, and quality improvement projects.
- Support in the investigation of Significant Events and complaints maintaining the register for these.
- Proactively ensure the recalling of patients is implemented and maintained by the admin team.
