**Role of the department**

The Community Nursing Service provides a comprehensive range of treatments which enable individuals to avoid unnecessary admission to hospital, or where hospitalisation is necessary, to support discharge back into the community. Nursing care includes the assessment and planning of care for patients, ensuring proactive advice and information.

**Job Summary**

To work within the Community Primary Care Network (PCN) to support and assist in the provision of holistic nursing care and support to patients, their families and carers. Your role will be pivotal within the local community by making a difference to our patients, delivering a high level of care in their own home.

**Main duties of the job**:
The role will involve providing care for end of life, wound management, administering medication, giving personal care, setting up equipment and recording observations within scope of practice and competencies.

**Detailed job description and main responsibilities**:
**Responsibilities & Results***

**Clinical**
- Coordinate and participate in specialist care e.g. wound care, Doppler testing, phlebotomy, bladder screening, medication and clinical activities, under the supervision and guidance of Registered Nurses
- Undertake diagnostic tests on request of District Nurses, Community Matron or GP’s e.g. for chronic disease monitoring programmes. This may include home visiting or responsibility for coordinating repeat or regular testing for residents in residential homes
- Undertake clinical interventions as delegated by a senior team member
- Participate in the assessment process of care needs and implementation of programmes of holistic care within set standards
- To be involved in supporting, educating and informing family/carers where appropriate
- Assist in the promotion of health and wellbeing of patients
- Work in partnership with patients ensuring privacy and dignity is maintained
- Be aware of and act upon, where necessary, the procedures in place to protect vulnerable individuals
- Be flexible to requests to move base temporarily or on a more permanent basis, as requested by service needs

**Essential criteria**:

- NVQ Level 3 or appropriate qualification or course
- English and Math GSCE C or equivalent

**Desirable criteria**:

- Experience in a health or social care setting
- Experience in a community Healthcare setting

**Skills /Abilities/ Knowledge**:
**Essential criteria**:

- Ability to provide quality care that is responsive to patients needs
- Ability to work independently following spoken or written instructions

**Desirable criteria**:

- Clinical skills e.g. Insulin administration, wound care, venepuncture, diagnostic tests
