This is a pilot role aiming to support individual residents with identified income support needs, including administering crisis support payments in liaison with Hackney Council. It is part of the City and Hackney Poverty Reduction Framework

This is an exciting opportunity to work closely with partners, to trial new and innovative ways of working that support the social determinants of health.

We are looking for an individual who is able to build trusted relationships both with practitioners and with residents, is open to trialling a new way of working and has the tenacity to unblock barriers.

**The people we are looking to fill this role will**:
Demonstrate an ability to be proactive and organised

Be a clear communicator and have an ability to communicate with different practitioners from different organisations

Be confident in engaging with residents providing support to them

Be able to work as a multi-agency team and enjoy working with a wide range of people

Be caring, dedicated, reliable and person-focused
