Supervise a team of Care Co-ordinators and drivers, ensuring excellent customer care skills are maintained and all duties completed Participate in induction, training and ongoing professional development of the Care Co-ordination Team, monitoring sickness and absence and supporting the operations manager with back to work interviews, sickness reviews and 1to1s Supporting leadership team with implementation of new services/processes Supporting with rota queries Ensure the smooth flow of patients through the department, supporting the care co-ordinators with: Patients requiring clinical assessment telephone/face to face Health care professionals requiring clinical advice/onward referral Co-ordinating DVT clinic Signing in/out all equipment/prescriptions/drugs key Allocating rooms/breaks for all clinicians Providing follow up core primary care appointment booking Ensure that all local and nationally agreed targets are met across areas of responsibility In conjunction with the Clinical Lead, monitor and manage the patient flow within the each pathway, ensuring that excessive waiting times do not occur. This may require flexing of resources in order to meet demand across services/pathways Accurately record on all relevant clinical systems ensuring all relevant data for each contact and direct appropriately Where demographics are incomplete, complete all necessary checks eg. NHS Spine to ensure most up to date records are available Undertake calls to patients Ensure that all staff follow the failed encounter procedure across all services Co-ordinate the arrangement of any follow-up care as directed by the clinical team Liaise with NHS 111 Service and other providers regarding patients that are out of area, duplicate calls or incomplete information Facilitate the electronic transfer of data both internally and externally ensuring that information governance rules are followed at all times Have a full understanding of the clerical backup system to facilitate the smooth transaction from a computer based system to a paper based system Undertake all administration duties associated with the role Act as the IT co-ordinator, troubleshooting problems with computers, printers and faxes. Ensuring that all password problems where appropriate are dealt with in a prompt manner.

Escalating any problems that occur with the computer system When necessary, contact staff to ensure all shifts are filled to ensure the smooth running of the service Understanding of the posts responsibilities in the business continuity plans Ensure regular attendance at operational meetings all training deemed as mandatory to be completed within the specified time frames Perform any tasks as requested that are within the boundaries of the role Ensure that all gtd policies and procedures form the basis of all decision making Ensure any issues highlighted on shift are escalated as appropriate to the Clinical Lead and fed into handover reports to accurately reflect patient flow and issues within the shift Ensure any actions contained within the handover report pertinent to the role are carried out within the shift
