This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke. We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join our Stroke Recovery Service based in the Durham area.
The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.
A key part of the role will be the delivery of 6-month post-stroke reviews for stroke survivors.
Reporting to the Stroke Association Support Manager, the Stroke Association Support Coordinator will:
- Support new stroke survivors and their carers from hospital discharge into the community.
- Deliver 6-month post-stroke reviews.
- Provide personalised information, advice and support.
- Support clients to make informed lifestyle changes which will help them to prevent further strokes.
The post holder will have experience/background in:
- A caring profession ideally with experience in supporting people with disabilities.
- Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
- Ability to use basic Microsoft systems.
To fulfill the role, you must be a resident of the UK and have the right to work in the UK.
**Our services are contracted, we currently have funding for this contract until 31 March 2024.**
