A fantastic opportunity has arisen for a for a secondment as a Band 4 Therapy Assistant practitioner to join our well-established team at Airedale Centre for Mental Health.
The vacancy is based on Bracken ward, anacute psychiatric ward, caring for males and females aged 65 and above who are experiencing acute mental illness.
As a therapy assistant practitioner, you will work alongside the occupational therapist and physiotherapist, assisting and where appropriate leading in the assessments of functional ability, deliver individualised treatment plans and support recovery and transition from hospital to home.

**Main duties**:
**Responsible for ensuring that they**:
1. Facilitate the falls risk assessments and management of falls risk for
patients admitted to the Older People’s ward and contribute information to
support MDT led decision making and implementation of individually
tailored falls risk care plans
2. Assess and prescribe walking aids for patients on Bracken ward and teach
how to use these effectively
3. Undertake routine physiotherapy assessments, use assessment findings to
devise and implement routine physiotherapy treatment care plans
4. Undertake basic occupational therapy assessments and implement basic
treatment plans under direction of an occupational therapist

5. Support the multidisciplinary team on Bracken Ward to gather a detailed life
care and support
6. Set up, lead and facilitate patients to participate in physical activity and
exercise groups on Bracken Ward
7. Guide B3 Therapy Support Workers to enable meaningful individualised
engagement for patients on Bracken Ward
8. Escalate more complex findings from therapy assessments to the specialist
physiotherapy or occupational therapists
9. Reports progress change or adverse events promptly to an appropriate
member of staff.
10. Document in line with the Trust Records Management policy all clinical
interactions with patients on Bracken Ward as a Therapy Assistant

11. Promote effective individual communication skills with patients
disseminating information
with colleagues
12. Promote, monitor and maintain health, safety and security in the workplace
13. Support carer focus groups, obtain and share appropriate feedback and
suggestions
14. Support therapist with equipment ordering

- Seek advice and support from your line manager whenever necessary.
6. Staff Supervision and Support
- Assist in the induction of new staff.
- Supports staff to attain developmental objectives as identified through the Trust
