The cardiac rehab team has an amazing opportunity to make a big difference in a patient’s life in such a positive way that it is a true privilege.

An essential but very rewarding aspect of the role is to quickly gain rapport with a patient and their families so that we can promote self-care and offer individualised care. This means we are fortunate in being able to offer patients the time they need which can be a rare commodity in the current NHS.

The role is varied and interesting, we offer flexibility to our patients and our employees as much as possible with patient care being offered in their own homes, clinics, community venues, video calls and the hospital.

Cardiac Rehabilitation Specialist Nurse team - Airedale NHS Foundation Trust (airedaletrust.nhs.uk)
- Undertake clinical duties in the community for patients on the cardiac rehab caseload.
- Work independently in the community and as part of a team.
- Record patient information electronically and report back to the cardiac rehab specialist nurse.
- Provide admin and clinical support to the cardiac rehab team.
- Provide patient education and health promotion.
- Promote self-care and provide psychological support to patients and their families following cardiac events.
- Assist in the organisation and delivery of cardiac rehab exercise programmes.

Our cardiac rehab team is a small, friendly agile team working in various locations. We have a flexible working culture with a mix of community, office and home-based work. Our wider Community Specialist Nurse's Team are experts in their field and provide support for people within the beautiful areas of Airedale, Wharfedale and Craven. We work autonomously and as part of the multi-disciplinary team. Our team strives for high quality, safe care which is patient centred and focuses on healthy communities and people.
- Responsible for using Right Care values and behaviours.
- Communicating with patients, other healthcare professionals and team.
- Promote health and wellbeing and manage interventions.
- Self management of workload and clinical development.
- Report to the cardiac rehab specialist nurses.
