Care Team Leader

Responsibilities:

- Leading and supervising your shift
- Case managing residents and liaising with families, healthcare professionals and other stakeholders to ensure that the best care plans are created and delivered
- Administering and recording medication
- Carrying out risk assessments
- Working alongside care assistants to support residents with daily needs, ensuring residents remain as active and independent as possible
- Working closely with the management team to develop services
- Provide a fun, inclusive, and professional approach at all times
- Delegations - who will be doing what during the day.
- Check continence charts and fluid charts from previous day for any actions required.
- Make sure any incidents are recorded on incident report forms.
- Encourage healthy activities with the residents in addition to structure timetables to promote well-being.
- Check personal care is done in a timely manner and to a high standard throughout the day.
- Spot checks on the residents’ bedrooms to ensure they are being kept clean, tidy and organised.
- Ensure all residents food and fluid intake is suitable throughout the day
- Handover night shift with key information regarding the day.
- Reporting any complaints or compliments to managers, incidents, anything unusual
- Report any ill-health amongst Clients and make requests for GP / Professional visits if asked to.
- Undertake personal care as appropriate.

**About You**
- NVQ Level 3 in health and social care / working towards
- Be well-organised, IT literate and adept at allocating and delegating work appropriately.
- Proven ability to lead and supervise others.
- A good understanding of care planning processes
- Be experienced in or happy to undertake training on administering medication
- Above all, have a natural ability to help make the lives of others more fulfilling.
