To develop and deliver an agreed project/workstream within the Birmingham integrated Care Partnership specifically within the Intermediate Care theme. This role will work very closely with colleagues from the organisations that comprise BICP; namely BCC Adult Social Care, NHS provider trusts and NHS Birmingham and Solihull as well as a range of internal and external stakeholders within Birmingham.
- Supporting Single Co-Ordinator on the scoping and planning of workstreams/projects within programmes, including support in identifying key milestones, deliverables and benefits
- Support in defining the technical aspects required to deliver the programme (scope/resources/work breakdown structure/timelines/budget/key outputs)
- Work on benefits realisation for the agreed workstream/project, including agreeing shared measures and approaches to measurement
- Work with the relevant teams to prioritise transformation workstreams and resources required for delivery
- Lead on implementation of agreed workstreams/projects across organisations and teams within BICP
- Define and support establishment of forums and task & finish groups, clear governance arrangements, including lines of responsibility and accountability to develop and implement the required changes
- Establish and maintain effective working relationships both within the host organisation and other Birmingham health and Social care organisations
- Contribute to the reporting, documenting and monitoring of agreed workstreams/projects, including coordination, preparation and presentation of documents at appropriate boards
- Support in Identifying and managing risks relating to the workstream/project ensuring that appropriate mitigations are in place and risks escalated as necessary
- Support manager in liaising with finance colleagues to ensure the workstream/project is delivered within budget
- Delivering the rollout of new ways of working for an agreed workstream/project within the Intermediate Care transformation programme
- Supporting the development and management of a robust project plan, which sets out clear and achievable milestones and timeframes. Ensuring that key milestones are achieved within the agreed timelines and budgets.
- Developing and monitoring qualitative and quantitative benefits to ensure the required outcomes are met and taking responsibility for programme and project reporting requirements
- Working closely and effectively with internal and external partners including, other System Improvement officers, the System Improvement manager, clinical and managerial leads, Third Sector Partners, Local Authority leads, clinical teams and service users and carers.
