**Where you fit in**

This post presents an exciting opportunity to become frontline practitioner within the Dependency & Recovery service. Our Recovery Practitioners support a number of Service Users at any one time, providing ongoing assessments and management of each Service User’s needs, capacity, opportunity and motivation to rehabilitate. They deliver and access interventions that support rehabilitation, working in partnership with a range of local providers and taking account of available risk information.

As a Recovery Practitioner, you will work with Service Users to understand their specific needs, and help them build tailored action plans which become roadmaps to recovery. With responsibility for supporting Service Users in addressing their dependency and recovery needs, you will have a great opportunity to be a positive influence for change and to work with a wide range of partners to improve Service User’s prospects.

In addition to your role with Service Users, this role will also give you the opportunity to work and collaborate with a range of external partners from across the Voluntary and Criminal Justice sectors, allowing you the chance to develop your system knowledge and build relationships across the industry.

The role will be based in the community and will require travel across various delivery sites across London. It may also require some occasional travel to other regions to access training and develop links with partnership agencies.

**Main Duties & Accountabilities**

**As a **Recovery Practitioner **your main accountabilities will include**:

- Undertake structured assessments to understand the specific dependency and recovery needs of the service user & construct tailored action plans in line with their specific needs, complexity level, risk factors & protected characteristics.
- Develop, monitor, and evaluate action plans which are tailored to the service user’s dependency and recovery needs and required outcomes
- Provide appropriate specialist and targeted dependency and recovery support, and interventions on a 1:1 and group basis
- Provide advice, guidance and information on dependency and recovery provision and support available within the service user’s local authority and support the service user to access these provisions
- Sign-post service users to relevant agencies/organisations that will support them with their dependency and recovery needs
- Utilise resources available to you to address dependency and recovery needs
- Provide advice and guidance ensuring dependency and recovery issues do not prevent service users from living a purposeful life
- Carry out effective assessments, action planning and risk management
- Provide advocacy support for service user’s to engage with partner solutions
- Support young people to lead safe, happy and productive lives
- Utilise our evidence-based structured interventions, and co-design and deliver tailored specialist training/courses/interventions to develop service users’ dependency and recovery awareness and risk management plans
- Provide advice and support for them to better understand the risk of dependency and recovery
- Refer to/book/follow-up service user appointments with dependency and recovery organisations_ _
- Support service users to develop an overall recovery plan based on referral and action plan outcomes
- Use Action Planning to identify service user needs for dependency and recovery risks
- Liaise with the Probation Practitioner in relation to risk management
- Use strengths-based approaches to support the service user to address their needs.
- Design/deliver sessions to address dependency and recovery issues and support service user to manage these effectively

- Inform, consult, and involve service user in determining rehabilitation plans and activities.
