Care Co-ordinator

Working alongside the clinical and admin teams, the Care Co-ordinator will help patients in finding the right support with the right person, at the right time. Care Co-ordinators champion person-centred care and provide options and education to support patients in best utilising primary care services. This role will primarily assist the same day team in facilitating referrals to Push Doctor, CPCS, utilising IPCAS appointments and contacting patients with follow-up appointments. Care Co-ordinators support the co-ordination and efficient cascade of information to patients on behalf of the same day team allowing them to spend more time triaging and consulting with patients.
**Care Co-ordinators will be expected to**: Have a thorough knowledge of all practice procedures and to work in accordance of written policies Undertake a variety of administrative duties to assist in the smooth running of the practice including the provision of administrative support to clinical staff and other members of the practice team Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way Facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies Oversee and maintain the relevant units in KLINIK, ensuring cases are resolved in line with practice procedures and service levels set by the Practice Manager Complete other administrative tasks as directed by the Leadership
