**Role Overview**:
This job opportunity allows you work with our mental health and wellbeing professionals to provide a holistic support package to vulnerable young people aged 16 - 25 years who are homeless or at risk of homelessness within **North Tyneside**.

As **Mental Health and Substance Misuse Coordinator**, you will take the lead on developing creative and innovative initiatives that contribute to positive mental health outcomes and personal wellbeing for the young people we support.

**Key Responsibilities**:

- Provide screening, assessment and one to one need’s led interventions for clients within North Tyneside pathway services presenting with mental health and substance misuse issues with a goal of self-management and recovery.
- Undertake person-centered interviews which identify areas where the person wishes to see change and/or recovery and make an accurate assessment of risk to self and others.
- Make decisions on suitability of new referrals, organise assessment, signposting to appropriate services and facilitating referral to specialist services where required.
- Signpost young people with more complex mental health needs to specialist intervention.
- Work with the North East Mental Health and Wellbeing manager to develop strategies, structures and processes in conjunction that proactively empower clients with poor mental health including understanding on how to better navigate statutory mental health and substance misuse services and provide greater advocacy for those facing crisis

**What We Are Looking For - Person Specification**:

- Degree qualified (or equivalent experience) in relevant field (e.g. psychology, counselling, psychotherapy, substance misuse, restorative work or social work) or hold relevant experience from other work.
- Experience and knowledge of working with young people facing homelessness with complex mental health and substance misuse issues including those who have offended
- Experience of undertaking assessment of need to facilitate and coordinate interventions
- Knowledge of relevant networks in both statutory and community settings & building and managing relationships with partner organisations
- Demonstrate an understanding for the need to use evidence based psychological therapies and how it relates to the post
