Clinical Discharge Co-ordinators

We have an exciting opportunity to join the Discharge Planning Team working as a Clinical Discharge Co-ordinator based at Bedford Hospital.
The Discharge Team facilitate and promote the safe, timely and efficient discharge of patients from the Trust.
The role will be ward based working collaboratively alongside the Multidisciplinary team to assess, agree and plan patient discharge pathways.
The post holder will have previous experience in Discharge Planning within a community or acute setting and will provide expert knowledge, advice and support for complex discharges.
You should be well organised and have excellent communication skills, being able to use your own judgement to problem solve to resolve any discharge delays.
The Discharge Planning Team covers a 7 day service working from 08.00 till 18.00 (including weekends and Bank Holidays).
Lead and oversee the co-ordination of adult patient discharges
Ward based working as part of the Multidisciplinary Team
Actively participate in MDT Huddle and Board Rounds
To support ward teams in ensuring that all patients have a discharge plan developed within 24 hours of admission as appropriate
Ensure effective communication between the MDT in regards to patient discharge
Undertake patient assessment, collect necessary information, identify and allocate patient discharge pathways in collaboration with the MDT
To case manage and co-ordinate the discharge plans for patients requiring complex support
Work collaboratively with external partners from community and Adult social care to ensure discharges are safe and effective for the smooth transition into community care
Undertake capacity assessments as required
Undertake equipment assessments and ordering for discharge
Complete Discharge Notification documentation
Troubleshoot and escalate discharge delays
Facilitate and lead extended MDT meetings

Monitor and manage specialist discharge pathways including Rehab, Neuro and ABI pathways

To encourage patients, families and cares to be actively involved in the discharge planning process
Facilitate nursing handovers to care facilities and agencies prior to patient discharge
