The Home First service is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands NHS Partnership Foundation Trust (MPFT).

The service seeks to offer support to people ranging from those that need mínimal support for a time limited period to rebuild confidence, learn new skills and relearn skills through to people at the end of their life requiring skilled health & social care interventions. The service operates 7am to 10pm 365 days per year.

The post holder will be responsible for the scheduling and allocating care calls on the Complex Discharge Tracker System. They will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales. They will ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.

The successful applicants will receive direct line management and will be delegated team work priorities from the Home First Shift Leads. The post holder will be enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills.

Review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.

Dependent on capacity and demand of the Home First Teams ensure that any unmet referrals are immediately escalated for authorisation by the Team Leader for the individualised plan of care to be sourced from external partners.

Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.

Ensure that each and every change to a patient’s package of care (reductions, increases, cancellations, closures etc) is accurately input into CDT, and that every change has a corresponding written explanation recorded in CDT to document the reasons why the change was made to provide an audit trail.

Realign the allocated rosters and ensure that the allocated care calls are within the correct geographical areas to enable the team to demonstrate efficiency.

Actively work with the Shift Leads to find appropriate staff cover to cover unplanned sickness or emergency cover as required.

Ensure that all referral and information mail boxes are closely monitored with the appropriate actions implemented

- Participate in regular supervision, guidance and performance review to meet your own professional development to ensure that all staff are supported and equipped to deliver the service requirements.
- Support care workers to deliver a high standard of care to individuals requiring the Home First service in the community, as directed by their Support Plan.
- Ensure that you participate in all appropriate training, developed and skilled to fulfil their roles and maintain competence. Participate in staff appraisals and play an active role in team meetings.
- Ensure that all Support Workers are properly supported if they have encountered emotionally/contentious/difficult situations whilst working with frail and vulnerable older people in the community as directed by the Shift Lead.
- Work in partnership with the Shift Lead to manage day to day staff attendance and escalate concerns as appropriate.
- Support and assist the Home First Teams to ensure that the Complex Discharge Tracker remains up to date and is utilised as a live electronic system.
- Maintain appropriate filing systems and keep up to date
- Ordering from suppliers inclusive of PPE, Stationery, yellow folders/paperwork
- Inputting shifts and increases onto Healthroster and keeping up to date as advised by the shift lead.
