Do you want to lead and develop quality clinically led services for Allied Health Professionals working in South East Staffordshire D2A Reablement Service? We are looking for an experienced registered Occupational Therapist or Physiotherapist to provide specialist reablement knowledge, experience and skills to enhance the service delivery and lead others working within the clinical teams, growing talent and leading an evidence based quality service for the communities we serve.

You will work with complex reablement conditions who are discharged onto the Home First Discharge to Assess (D2A) pathway following a hospital admission. Clinical focus will be providing expert specialist clinical assessment, care planning and evaluation with a focus on rehabilitation supporting our Home First D2A/Reablement service in the rehabilitation of the communities we serve. The rehabilitation aim is to support individuals to maximise their potential and independence and reduce the need for long term care and support.
- Clinically leading reablement D2A teams and managing a complex Physiotherapy/Occupational Therapy caseload.
- Be a Clinical Lead Physiotherapist/Occupational Therapist and have a detailed professional and clinical knowledge. You will be an advanced clinical lead practitioner and have a focus on reablement and rehabilitation.
- Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support.
- Have supervisory, training and leadership responsibilities where appropriate and will have responsibility for team performance and service developments/improvements.
- Be responsible for a defined population of service users in a variety of settings across the integrated care system including the community and hospitals.
- Demonstrate initiative and provide a solution focused approach to challenges.
- Work with the operational lead undertaking delegated team management/leadership activities.

Working in a Multi-Disciplinary Team and in Partnership with Home First and other agencies you will:

- use expert knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider each service user’s personal circumstances and preferences in order to implement and adjust programmes of care and intervention.
- maintain and develop skills and knowledge to deliver a range of evidence based interventions and sets an example of good practice to other team members including skill sharing. Provides teaching/training in specialist area to the wider workforce.
- maintain contemporaneous and accurate records as required by MPFT, legislation and in line with professional standards (HCPC and CSP/RCOT). Support identified team members giving guidance and direction where appropriate.
- ensure that the skills of identified team members are appropriately deployed and that care is delivered by staff with the relevant education, expertise and competence.
