Our fabulous #TeamSBDS are hiring now for energetic, empathic, forward thinking, flexible and supportive individuals to join our established teams providing person led, holistic Day Support as Support Practitioners. We have up to 30 hour contracts available along with relief opportunities if you're looking for a bit more flexibility.

**The Role**

Our vibrant and dynamic service comprising of three highly trained teams. who provides complex support along side vocational opportunities to adults with physical and learning disabilities both at our hub and out in community settings. Our Connects Hub provide complex support to individuals who come to our building. Connecting Communities provide blended support within our building and promote opportunities within the local community. Community Experience are all about local opportunities within the community and enable the people we support to attend.

You will be based within one of our three Teams;
- Connects Hub-Critical Complex Needs building support.
- Connecting Communities-a blend of building support and community social inclusion.
- Community Experience-complete community social inclusion.

The people we support are fun loving and enjoy being out and about, you will support them to be involved in their community, and participate in their hobbies and interests, which include watching the rugby, football and wrestling, keeping fit, going swimming, knitting, gardening and cooking. Our aim is to enable the people we support to live a fulfilling and valued life, where they can achieve their own personal goals and aspirations. You will take an active role in the planning and delivery of person-centred care and encourage the people we support to express their views and wishes within their personal plan, which you will keep up to date and relevant.

- Full UK driving licence (min. 1 year driving experience)

Shift Patterns

Our dedicated and friendly team provide support support between the hours of 08:00 - 22:00, seven days a week.
