We have a new and exciting opportunity within the General Respiratory team for a FT, experienced MDT Co-ordinator, band 4 within the ILD Service.

All patients pathways will need to be monitored and the post holder will need to ensure any actions from the MDT are actioned. It is also an opportunity to support gathering information on patient experience via friends and family feedback and provide data to support compliance with this national programme.

To have an understanding of patients’ individual needs and personalised plans of care for ILD service following review from the clinical teams ensuring multi-disciplinary, patient centred care.

Undertake regular reviews of the patient journey and implement any changes to the patient’s pathway suggested by the clinicians.

Ensure appointments are coordinated according to MDT recommendations, minimising the number of patients hospitals visits as appropriate.

Monitor investigations and tests to ensure they have been completed and results/reports are available prior to appointments and consultations and MDT meetings with the clinical team.

Communicate with the clinical team to monitor the progress of all patients according to the ILD Service Pathways in line with local and national standards whilst identifying and minimising any delays.

Promote effective inter departmental relationships with departments to ensure the ILD patient journey is efficient and effective.

Undertake and promote practice sensitive to the needs of patients and families from multi-cultural backgrounds.

Work with clinical and non-clinical colleagues to identify and develop administrative processes within the ILD Service.

Monitor and review patient waiting lists, ensuring patients are booked into the appropriate outpatient clinical setting as per Standard Operating Procedure.

To undertake any other duties which are deemed appropriate to the band when requested by Senior Staff
