Oxleas Adult Autism Service is appointing an Autism Liaison Worker to join a dynamic team committed to supporting the provision of high quality health care to Autistic people living in the London Borough of Bexley.

The Autism Liaison Worker post, based at Queen Mary’s Hospital, will work on the post-diagnostic pathway in close relationship with the Multi-disciplinary Team. The Autism Liaison Worker will be pivotal in capturing the person’s non-medical and non-clinical support needs with the help of the assessing MDT and then navigating and sign posting them to the appropriate community third sector service and provide information on how to access them. The Autism Liaison Worker will guide the person through the community offer, make relevant contact if needed and support in attending key appointments.

The liaison worker and the Oxleas Adult Autism Service will not act as a care coordinator and will not be responsible for managing the risk of the diagnosed person. This service will not have a clinical and/or care profile, it will only be a support service for bridging the gap for person post-diagnosis from the ASD diagnostic service to the correct services available in the community.
- To provide person-centred support to people who have been assessed with autism spectrum disorder and require access to the third sector services in Bexley and help them to liaise and connect with the appropriate service.
- To improve people’s experience and quality of care within the borough of Bexley by providing a point of contact for non-clinical queries and sign-posting and support communication with available post-diagnostic services.
- To be part of the ASD diagnostic pathway and understand guidance and operating procedures.
- To inform colleagues of the potential third sector services to which the person can be referred to and which can support people to self-manage.
- To assist people who may be eligible to access community services.
