The Primary Care mental health Liaison Worker will provide direct clinical input and case finding, liaison and training into one of four locality Community Health Services Locality Teams working alongside, district nurses, therapists and care navigators and the attached GP practices to provide services to patients on the integrated care register (frailty) across the borough of Tower Hamlets.

Support the integrated care model to identify adults at risk of admission.

Support the identification of mental health needs of people with long term conditions who are identified as high risk.

Deliver assessment and goal focused/time limited interventions to the identified above whose needs are not being meet by specialist/mental health services.

Support referrals into borough wide mental health services if more extended / in-depth interventions are needed.

Develop strong working relationships with primary care to ensure good communication and care planning of individual and population care and proactive addressing of mental health needs

Develop relationships with care homes and extra care sheltered schemes to proactively support older adults at risk of hospital admission and have mental health needs.

Promote improved awareness and knowledge of mental health and drug and alcohol problems and their impact, treatment and inter relationship amongst the broader integrated community health team and GP’s, though formal training and joint working and support.
