This is an exciting opportunity in our Quality Improvement Team for the right person wishing to develop their skills in a fast-moving Primary Care environment, in the heart of the community.
- One _Norwich Practices teams have a wealth of experience of working in primary care. This role provides the opportunity for the successful applicant to develop their knowledge and skills within the local health and care system, while working in general practice making a difference to our patients.

We are looking to recruit an enthusiastic and dynamic Care Coordinator to proactively work with people with diagnosed long term conditions to help them more effectively manage their own health. The Care Coordinators co-produce a personalised care and support plan for each patient, making onward referrals to specialist teams as required. Care Coordinators develop a sound knowledge of local health, community and social care provision as well as voluntary sector support.

The role of the Care Coordinator within General Practice is recognised as vital to create resilient primary care teams, able to provide an integrated, patient-centred service.
