There is a new opportunity for a lung cancer pathway navigator within the PRUH lung cancer diagnostic service. The post holder would be responsible for the co-ordinating and navigating patient’s progress through the lung “two-week wait” suspected cancer pathway. The pathway coordinator will be a key member of the team in establishing the lung rapid diagnostic clinic.

The pathway navigator will work with and support other team members including Consultants and Clinical Nurse Specialists (CNS) to aid in rapid diagnosis of lung cancer and meet National Optimum Lung Cancer Pathway (NOLCP) time frames.

The post-holder/s must be able to work independently and follow the policies and procedures for the department. The post-holder will be expected to use their own initiative and keep their line manager and senior colleagues informed and escalate delays in pathways.

The role will have lots of variety and ad hoc demands and the post-holder must possess excellent time management and planning skills. The post may develop over time to meet the specific needs of the pathway and the changing demands on the service.
- Lead and support consultant triage of referrals
- Coordinating scan and clinic appointments for patients
- Proactive management and tracking of the patients’ pathways
- Ensure diagnostic are tests booked and attended and tracked on PTL
- Working with Consultants and the MDT coordinator to coordinate and streamline referrals to the service and ensure appropriate appointments and investigations are arranged promptly
- Communicating outcomes of triage decisions to patients
- To communicate with patients to ensure they are aware of upcoming appointments, and that any barriers to patients attending are minimised. Working with CNS team to achieve this
- Working with the MDT coordinator to ensure patients’ journeys through the cancer diagnostic pathways are rapid, and that any delays are mitigated. The Pathway Navigator should proactively identify any delays and take measures to reduce them
- Ensure diagnostic are tests booked and attended and tracked on Patient Tracking Lists (PTL)
- Be trained in booking systems for radiology, clinics and respiratory so that appointments can be booked independently
- Update tracking lists to ensure team is aware of progress
- Attend weekly PTL meeting and contribute to progressing patients diagnostic pathways
- Attend MDT meetings on a weekly basis. This may require being available for an early start, late finish or during lunchtimes.

The pathway navigator will work with and support other team members including Consultants and Clinical Nurse Specialists (CNS) to aid in rapid diagnosis of lung cancer and meet National Optimum Lung Cancer Pathway (NOLCP) timeframes.

The post holder has a general duty of care for the health, safety and well-being of work colleagues, visitors and patients within the hospital in addition to any specific risk management or clinical governance accountabilities associated with this post.
