May be required to work without supervision in remote locations To provide experienced Dietetic support and advice to patients and their carers Undertake dietary review and nutritional assessment (including consideration of anthropometry, biochemistry, pharmacology, and medical history) of patients and set a plan of care for both in-patient and outpatient care within this remit. To record accurately all assessments and interventions in medical notes/ nursing documentation/dietetic record cards in line with Trust policy Deliver Dietetic lead outpatient sessions for patients with a wide variety of complex needs, to assess patients nutritional status, therapeutic dietary needs, and advice regarding dietary/lifestyle changes. Review progress regarding dietary advice given where appropriate. Communicate in writing to the referrer noting nutritional status of patient and outlining lifestyle and dietary plan given and the follow-up arrangements, maintaining accurate, comprehensive, and up-to-date documentation, in line with legal and departmental requirements.

To effectively communicate a plan of care with other appropriate health-professionals e g. Community Dietitians, GPs, clinical nurse specialists consultants etc. To work collaboratively with other disciplines, ensuring delivery of a co-ordinated multidisciplinary services. To review and develop a range of Specialist educational resources which have a clear evidence base and which are appropriate for the users of the services.

To develop a network of communication between other agencies e.g. catering, pharmacy, GPs, consultants, nurses and other allied health professionals etc. Work as a senior autonomous practitioner liaising with other specialist practitioners on a local, Strategic Health Authority and National level to promote equitable, evidence-based practice. To communicate effectively with patients and carers to maximise condition management, recovery potential, further disease prevention.

To recognise that individual patients have different needs for information at different phases of their illness and to be responsive to patients needs. Communication skills of active listening, emotional engagement, empathy, tact, openness, motivation, and reassurance will be used with a variety of patients. Barriers to effective communication will regularly be evident e.g. anxiety, fear, altered perceptions, impaired sight and impaired hearing.

To have the ability to deal appropriately with daily exposure to distressing or emotional circumstances. To be sensitive to patients and carers spiritual needs and to be aware of local resources for spiritual care. To be able to recognise psychological needs of patients and be able to offer general emotional care and know when to refer patients to a more specialist service.
