Support Time and Recovery Worker

The post holder is required to work within an integrated MDT focusing on older adults suffering with a wide range of mental health problems. The post holder will work with individuals who are in crisis and having difficulty coping, to prevent hospital admission where there is potential for providing acute intervention packages enabling the individual to remain at home or within a care home environment.
The post holder will be involved in assessing patients needs, planning and implementing the plan of care.
This is an exciting opportunity for an experienced Support time Recovery worker wishing to join the mental health team in Anglesey and Gwynedd. Applications are welcomed from enthusiastic practitioners who want to make a difference in primary care service provision. You will have excellent communication skills, be self-motivated, proactive and committed to promoting innovation for service delivery.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
