Position: Rapid Response Worker (Pilot Project)
**Salary**: £12.69 per hour
Hours: 37.5 per week
Contract: Fixed Term (12 months)
Location: Gainsborough & Surrounding Area
Linkage Community Trust is looking for a Rapid Response Worker to join our team as we develop a new and exciting service in Gainsborough.

This new role is an opportunity to join our care and support team as we continue to invest in the development of services within Lincolnshire and specifically this new location of Gainsborough.

Who are You?
- You are passionate about quality and committed to providing high quality care

- You are able to remain calm in challenging situations with a considerate and understanding approach towards to needs of the individuals with learning disabilities and/or autism
- You are an established professional working in a Learning disability setting with good knowledge of undertaking assessments, creating care plans and identifying risks to ensure that individuals independence is promoted and managed in a positive way
About the role:
- You will work as part of the rapid response team, providing short term care and support to people with a Learning disability and/or Autism
- You will facilitate discharges from hospital to residential settings or to the individuals own home, and/or prevent in patient admissions where possible
- You will contribute to and undertake initial assessments for individuals ensuring that person centred support plans are implemented and any identified risks are managed positively to promote independence
- You will support the service in providing a swift response to service requests and to act as the co-ordinator between the person needing the support and other organisations and agencies as needed.
- You will be flexible to requests for assessments and support, providing engagement at times that may be outside of core hours
- You will have experience of working with service commissioners, acting in a responsive manner upon receipt of referrals, and working flexibly to meet the needs of the individuals accessing the service
- You will be confident in maintaining accurate and detailed electronic records and have good IT skills
