**About the Role**:
Chelmer Medical Partnership has an opportunity for a care co-ordinator to support our 3 GP practices across Chelmsford and to assist in re-shaping primary care across our communities. The role is offered on a full or part time basis.

The care co-ordinator will support the team with the day-to-day administration of running of reports, providing support to our QOF team, meeting with patients to support their care and assisting the clinical team.

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

Previous experience of working in primary care and knowledge of SystmOne would be desirable but not essential as full training will be provided.
