**JOB TITLE**: Mental Health Mobile Hub Support Worker

**SALARY**: £28,560
**HOURS**: 37.5 hours per week
**LENGTH**: 13 months fixed-term contract until July 2025

**Context of the role**:
We are excited to be recruiting for an EMHIP Mobile Hub Support Worker, which is a role that has been co-designed with individuals with lived experience of mental health care to provide direct, person-centred support to individuals in their local communities.

This role is part of the Ethnicity Mental Health Improvement Project, an innovative, community-led project, originating in Wandsworth and now rolling out to Boroughs across South-West London, including Croydon. The goal of this project is to reduce the health inequalities that people from ethnic minorities face with Mental Health Services.

We are looking to impact on outcomes such as,
- Black people are 4.6 times more likely to be detained under the Mental Health Act.
- Children from a BME background wait 1.6x longer than white peers, for initial access to mental health services.

This role sits within the Mental Health Mobile Hub team, an all-age, whole-family hub service, delivered within both faith-based and community-based organisations across Croydon. You will join a passionate team of experienced, culturally aware support workers and clinical staff, embedded in the community to provide services to all local residents, but with a specific focus on BME communities.

The project is led by the Croydon BME Forum in partnership with the Asian Resource Centre Croydon and South West London Integrated Care System (SWL ICS) and South London and Maudsley NHS Foundation Trust (SLaM).

**Job Summary**:
The Mobile Hub Team Support Worker is a skilled, knowledgeable, and empathetic person, who enjoys working with people and taking a person-centred approach. They must be flexible, adaptable, and comfortable working in an active setting, with people and professionals across the health and social care system. They will be practical, resilient, well organised and have excellent communication skills.

You will work in a variety of settings, including:

- Faith-based groups
- Community settings
- Colleges
- BME Forum Head Office

**Main responsibilities**
- Deliver one-to-one support to hub users and manage a caseload of individuals as agreed with the Hub Team Manager.
- Support new referrals and offer a person-centred, well-being assessment that identifies their well-being difficulties and develops a plan to support positive mental and general well-being.
- Provide general advice on, but not limited to,
- Welfare Benefits, including Universal Credit
- Housing.
- Money and debt management.
- Employment opportunities and training support.
- Support access to, and the delivery of, a variety of peer and social support groups, workshops and activities.
- Facilitate direct referrals to secondary care services, community services, or to hub team psychologists.
- Attend training, forums, and meetings to ensure knowledge remains relevant and up to date.
- Provide updates and feedback on clients and the Mobile hubs informally and through written reports.
- Maintain accurate and up-to-date service user records using online database systems.
- Contribute to the continued development of EMHIP by providing ideas and input on how the project can be improved.
- Support and own the running of our mobile hubs, building relationships with host organisations and managing timings.
- Support with outreach and raising awareness of EMHIP & the Mobile hubs, through the creation of marketing material, canvassing, presenting to groups and organisations, and online activities.
- Occasionally working alone and visiting people in community settings.

- Relevant mental health, social care or psychology-related training or qualifications.

- Demonstrable evidence of equivalent learning, training or experience.
