**JOB DESCRIPTION**

**POST**:Lead Young CarersSupport Practitioner

**REPORTS TO**: Operations Manager

**GRADE & SALARY**: £24,850.00 (FTE)

**HOURS**: 29.6 hours per week

**LOCATION**: South Lakeland
- Kendal
- Office based

**PURPOSE AND KEY OBJECTIVES**
- Meet the outcomes identified in Westmorland and Furness All Age Carers Service, and other Grant Funded Bids
- Support the integration of services for Carers through a whole family approach, improving access to local support, building and promoting existing networks for Carers across South Lakeland.
- Provide information, practical advice and emotional support to all Carers so they feel less isolated, develop strategies with the Carer to manage their own health and wellbeing to enable them to continue in their caring role
- Contribute to the development and ongoing work of Carer Support South Lakes

**MAIN TASKS AND RESPONSIBILITIES**
- To line manage all staff working within contract and grants associated with this post.
- To provide detailed outcomes, identifying the difference that we make to our Carers
- To support the identification of Carers and ensure that the formal assessment of their individual needs and outcomes are met, that are identified in the assessment process.
- To support and manage all Students placements.
- Establish care and support pathways that deliver integrated and collaborative interventions by the multi-disciplinary team that benefits the whole family approach to care
- Develop networks with an appropriate range of key services and agencies, to include but not limited to: Primary Care services, Mental Health services, Hospital Discharge Team, ICC (Integrated Care Communities), Relevant voluntary sectors, supplier agencies and Community Development Services.
- Identify a panel of Carers who will become Experts through Experience who will contribute to Implementation of Services, Service delivery and Evaluation
- To perform the Duty Manager role when requested.
- To inform safeguard lead of all alerts in relation to safeguard, near misses, accidents and health and safety issues
- Empower Carers to achieve improved quality of life and health and wellbeing outcome, to build resilience to have a strong voice in the delivery of the assessment review and evaluation process
- To work in accordance with the Safeguarding Adults at Risk Protocol and CSCP Safeguarding Protocol
- To refer or signpost Carers to other organisations which can provide services for the Carer
- To role manage Volunteers allocated to your team
- To carry out other such relevant duties as are required by this post.
- Identify Carers and undertake a formal assessment of their individual needs in accordance with assessment criteria defined by Westmorland and Furness Adult Social Care
- Formulate a support plan tailored to meet the Carers needs
- Provide telephone or face to face support for Carers requiring emotional support
- To establish and maintain all professional standard case notes on individual Carers and to complete all database requirements (both internal and external) for record keeping and monitoring.
- To work with all statutory and voluntary agencies and liaise with all health and social care professionals in support of the health and wellbeing of individual Carers.
- To develop and maintain links with Statutory Authorities and other agencies to raise awareness of Carers issues and their needs.
- To provide information and support to Carers, whilst promoting their independence.
- To refer or signpost Carers to other organisations which can provide services for the Carer
- To support in the delivery of trips, activities, residentials and social groups
- To attend carers events, meetings, forums and deliver presentations as required
- To carry out other such relevant duties as are required by this post.

**ORGANISATIONAL REQUIREMENTS OF EMPLOYEES**

- Contribute to the overall achievement of CSSL mission and objectives
- Ensure the values of CSSL are upheld across the organisation
- Assist CSSL in Community Fundraising initiatives
- Promote CSSL social enterprise initiatives
- Participate in fundraising activities with and on behalf of CSSL
- Participate as part of the requirements of CSSL to meeting their ISO 9001 Quality Award standards
- Provide a supportive working environment to all staff and Volunteers
- Attend regular supervisions, staff and team meetings as requested
- Contribute to research and reports
