Patient Access Navigator

This is an exciting opportunity to progress the Health Inequalities agenda within the Trust, delivering a digital inclusion pilot and implementing interventions that address inequalities in access, care and outcomes, creating sustainable change for patients. The Patient Access Navigator ( Digital Inclusion) is a new post with the specific purpose to support those patients more likely to experience blocks to how they access or experience care. The aim of the role is to provide equitable access to care for all our patients and advancing the NHS Digital Inclusion strategy.
The role requires someone with exceptional organisational skills who can also demonstrate great communication, analytical, interpersonal and networking skills. A caring and sensitive attitude is essential as you will be communicating with patients, their families and carers.
The post holder will work closely with colleagues across the Trust to identify opportunities for improving patients’ access to care and their experience and with our community partners to maximise opportunities to reach community groups and design coordinated strategies.
