An exciting opportunity has arisen to join the Emergency Department (ED) at the Leicester Royal Infirmary, to work as an Emergency Department tracker in Adult and Children's Emergency Department.

University Hospitals of Leicester, Emergency Department can take an average of 700 patients through its doors on a daily basis, so we require a motivated, well organised person who loves a very busy environment.

The tracker team enhance the team’s visibility and skills in managing and strengthening the patient flow, experience and supporting our clinical team.

Previous knowledge of working in an NHS Emergency Department and having an insight into patient flow would be an advantage. You will be primarily responsible for coordinating, communicating and organizing the effective, appropriate and timely process of patients throughout their Emergency Department stay, knowledge of Emergency Department Targets will be essential.

You will have excellent communication skills, be able to work on your own initiative as well as within a team and be able to prioritise and organise your workload. You should have relevant years clerical experience with knowledge of the Data Protection Act. Flexibility is a must within this role.

Your working hours would be on a 24 hour shift rotation, 7 days a week, 365 days a year. Part Time hours will be considered.

You will be an integral part of the operational team working within the Emergency Department, supporting patient flow, alongside the Flow Manager and Manager of the Day.

You will be joining an existing team of Trackers, with a wealth of experience within the role and across the department.

You will be working alongside the ED Flow Manager, Manager of the Day, Nurse in charge and Doctor in Charge to support the flow of patients through our department, covering all areas such as Ambulance Assessment, Emergency Room, Majors, Injuries, Childrens and our Minor Illness and Injuries area (MIaMI).

1. It is the responsibility of the post holder to ensure that accurate and up-to-date patient and administrative details are readily available on the Emergency Department computer system. The post holder must effectively communicate with the patients’ named clinician to ensure they are aware of the patient situation on a continual basis.
2. The post holder will be responsible for ensuring the smooth flow of patients within all areas of the Emergency Department and to support the Nurse in Charge and Doctor in Charge by chasing patient plans and next steps (radiology, pathology, etc) to minimise waiting times (under 4 hours).
3. It is the responsibility of the post holder to ensure that all necessary patient administration data is captured to enable an appropriate and timely admission, transfer or discharge of the patient from ED. This will involve close liaison with the acute assessment units flow co-ordinator/bed coordinators.
4. It is the responsibility of the post holder to ensure that the acute assessment unit flow co-ordinators are directly made aware of all the patients that require admission to a hospital bed as soon as the patient is identified for admission. The post holder is responsible for liaising with the clinical team to identify the priority of the patients to be admitted within the Trust timeframes, ensuring that the wards are aware of the patients
being transferred to them.
5. The post holder should work with the departments nursing and healthcare assistants to ensure that all tests are sent to Pathology and to ensure the clinicians are aware when the results are available, to ensure that the results are reported in a timely manner, and escalated to the ED flow manager if this is not happening.
