Urgent Care Response Team Allocator

A level 3 UCR Team allocator requires knowledge of facts, principles, processes and general concepts in a field of work. They will contribute to service development and are responsible for self development’. (Skills for Health 2009)
The Urgent Community Response Team (UCR) is an integrated Health and Social Care Team within Calderdale Community. UCR delivers a seven day service with staff working on a rota basis for different shift patterns covering the whole of Calderdale.
The service focuses on the delivery of rapid assessment and intervention for adults of all ages who do not require specialist acute hospital admission but who require crisis intervention within a community setting. This may be due to an alteration in their physical or psychological well-being, or a temporary change in their social circumstances which makes it difficult for them to be maintained in their own home without a short period of rehabilitation or intervention from UCR.
UCR Team allocator will contribute and facilitate the day to day running of the team, allocation of work load, ordering essential supplies.
- The post holder will provide office based support for the Urgent Community Response Team
- Co-ordinate and prioritise referrals into Urgent care response from both acute and primary care
- Work proactively within the Urgent community Response team to support within an extremely busy office environment
- Work flexibly to meet the needs of the service
- The post holder will be a key member of the urgent community response team and will contribute to the delivery of a quality service and maintenance of quality standards

- To co-ordinate Referrals Received within the Urgent Community Response Team
- To communicate effectively and in a timely manner, with relevant clinical staff and other colleagues: to ensure referrals are actioned within the appropriate time frame.
- Maintain and distribute documents, including reports, spreadsheets and data basis
- Participate in service audits where appropriate
- Order and receive clinical supplies, including stock taking where necessary
- Ability to work autonomously, plan, prioritize and organize own workload, whilst meeting specific deadlines of the team
- Maintain strict confidentiality in conjunction with Data Protection act
- To be flexible to changes in service needs and undertake training as and when necessary
- To carry out any administrative functions as requested including referral to other services
