End of Life Coordinator

An exciting opportunity has arisen to work with the End of Life Coordination team at HDFT. The end of life coordination service works to deliver an effective integrated multidisciplinary community based end of life service which provides care for patients aged 18+ years requiring domiciliary end of life care, complex symptom management and assessment of their changing condition and / or disease progression.

To receive referrals and provide a single point of contact for community end of life services, GPs, acute trust colleagues, patients and families and service providers across Harrogate and Rural District.

To ensure co-ordination and communication of end of life care, ensuring key services and processes are in place, care packages and equipment are sourced and end of life care needs are regularly reviewed.

To liaise with care providers and, after registered nurse review, redeploy resources as appropriate

To end the episode of care either by onward referral or discharge with appropriate notification and cancellation of equipment and care packages.

To operate within the hours of 9 - 5, 7 days a week with out of hours holding visits provided by the community care teams if required.

This role provides a great insight into the NHS and requires excellent inter-personal skills when communicating with patients and members of staff at all levels within the Trust.
