**Social Care Support Officer (Reablement) Role Profile
As a Reablement 'Social Care Support Officer', your main role is to be the main point of contact for a number of allocated individuals in receipt of Reablement. The main tasks will include:
Conducting 'Initial Contact' for new Reablement individuals and updating 'Reablement Plans' if required. This will include explaining Reablement to the individual and / or relatives, reviewing current Reablement goals, and amending these where identified

Continually reviewing individuals' progress throughout their 'Reablement Journey' to maintain capacity within the 'Service'

Adopting both a therapeutic and preventative approach, utilising alternatives to formal care support (e.g., 'Assistive Technology', equipment, 'Voluntary Services' etcetera)

Undertaking 'Care Act' assessments and 'Support Planning' when identified as appropriate

Attending weekly 'Team Meetings'

Supporting colleagues to manage capacity across all Reablement Teams

Working closely with the 'Reablement Provider' and other colleagues (i.e., within both 'Health' and 'Social Care')

This position is part time, please note the salary quoted is full time equivalent, the salary received will be paid on a pro-rata basis.
