Local Care Direct is pleased to offer a fixed term position for a **Telephone Assessment Clinician** at our Clinical Hub, working mainly within the Urgent Community Response (UCR) service.

**Hourly Rate**: £22.43 to £27.90 (Depending on Qualifications and Experience) for working In Hours + 30% enhancement for working Out of Hours

(In Hours = Monday to Friday 08.00 to 18.00 & Out of Hours = Monday to Friday 18.00 to 08.00 and all-day Saturday & Sunday)

**This position is fixed-term for up to 12 months.**

**Shifts available (working across a 4 week rolling rota)**:

- Monday weekly 14:00-20:00
- Tuesday weeks 1,2&3 10:00-18:00
- Tuesday week 4 8:00-14:00
- Thursday weekly 08:00-20:00
- Friday weekly 08:00-20:00

**Base location**: Local Care Direct, Unit 2, Longbow Close, Huddersfield, Bradley, HD2 1GQ

**Main duties of the job**:
The Telephone Assessment Clinician will perform remote consultations for acutely unwell patients.

To fill this exciting role, you will be an experienced advanced clinician (e.g. Physician Associate, Advanced Practitioner or Urgent Care Practitioner) with an interest in complex clinical triage and patient-centred care. You will have experience of making care plans with patients, often with those of a complex nature.

The UCR hub delivers care to acutely unwell patients within a 2-hr timeframe either by a visit from a face-to-face clinician from our partner services, or with advice, prescriptions, or signposting to other services. We work closely with community and hospital services, as well as Yorkshire Ambulance Service.

The UCR hub provides a rapid intervention to provide the support residents need to remain independent and avoid an admission to hospital.

There is also the opportunity to work in our other Clinical Hub services which are rapidly developing, all of which are focused on providing high quality remote urgent and primary care to patients.
