**Role Summary**
**Responsibilities**
You will be required to meet contractual and grant requirements by delivering information, advice, and support services in a one to one or group setting.
You will work closely with volunteers to enhance service delivery.
You will engage with service users on a formal and informal level to ensure they are involved in the coproduction of service delivery.
You will be required to work closely with your line manager to contribute towards developing plans, monitoring, evaluating, and producing regular reports.
You will be accountable for the use of charitable funding.
You will be an active member of the Improving Lives Plymouth team working closely with peers to develop cross project delivery to provide a wraparound service to our service users reducing the need for them share their story when moving between different teams within the organisation.
You will be required to assist with the implementation of identified accreditation standards.
You will be required to undertake any additional training relevant to the role to contribute towards your continued professional development.
You will carry out any other tasks which may be within the scope of the post to ensure the effective delivery and the needs of the service.
**Key Tasks**
- Provide information, advice, and support to service users including those who have disabilities or mental health conditions in areas such as health and social care and welfare benefits.
- Liaise with statutory & voluntary agencies on behalf of service users and maintain positive relationships with them
- Assess need and create action plans with service users that focus on their choices, strengths, and responsibilities
- Maintain accurate records and manage a caseload to contribute towards project monitoring and reporting
- Visit service users either at home or in the community following a risk assessment
- Keep up to date with legislation, case law, policies and procedures relating to your work and attend appropriate training.
- Undertake other tasks that are appropriate to the role and job description.

**Experience of**
- Working with the complex needs of service users
- Advice, assessments, support planning and reviews
- Working within social care, education, voluntary or health and wellbeing sectors and the ability to achieve positive outcomes for individuals and vulnerable groups
- Working with external organisations
**Essential Skills**
- Ability to support individuals
- Ability to lead by example, promote change and demonstrate professional behaviours
- Excellent communication and inter-personal skills, with the ability to empathise with the needs of people using our services
- Ability to deal appropriately with confidential matters
- Ability to develop and maintain constructive relationships with professionals from other agencies and share information appropriately
- Excellent IT skills including the use of Microsoft Office and CRM systems
- Well-developed organisational and time management skills
- Experience of balancing conflicting workload priorities
- A positive, energetic and solution focussed approach
- Ability to work in stressful situations
- An understanding of what constitutes a safeguarding issue.

- Trauma informed approaches to service delivery
- Involving service users with project delivery
- Mental health issues, loneliness, long-term conditions, carers issues and veterans’ issues and the wider armed forces community, learning disability, frailty, sensory and other physical disabilities.

**Qualifications**
- Level 2 Information, Advice and Guidance, or above
- English and Maths qualification

**Job Type**: Part-time
**Salary**: £16,932.00 per year
