**Job summary**:
Are you passionate about older people's healthcare? Do you want to be part of a dynamic and exciting time for the delivery of older people's care across the acute and primary care system? We are seeking to recruit Advanced Clinical Practitioners (Band 8a) or trainee Advanced Clinical Practitioners (Band 7) to work within the Older Peoples Assessment and Liaison teams at University Hospitals Birmingham Foundation Trust. This post supports the nursing and medical teams in the Emergency Departments and Assessment Areas at all of our Acute Hospital sites. You will be working as part of the team providing leadership, expertise, advice, and support to these areas, ensuring the provision of high quality care to older people from the first point of contact through to admission and discharge. This exciting opportunity will include the delivery of a Comprehensive Geriatric Assessment process providing early assessment of patients, interpretation of investigations, consultation, independent prescribing, and liaison with medical, nursing, allied health professionals and support teams across the acute and primary care interface to maximise functional independence and support a "home first" approach to care.

**Main duties, tasks & skills required**:
The post-holder will be expected to exercise a high degree of personal professional autonomy and integrity to make critical judgments to satisfy the expectations and demands of the role. In addition, the role requires excellent communication skills and the ability to discuss highly sensitive matters to provide excellent care.

The service is currently operating from 0800-2000 Monday to Friday and 0800-1600 Saturday and Sunday however post holders will be required to work a variety of shifts as the service develops potentially covering a 7 day rota with extended hours and across all sites.

Post Graduate Qualification in specific

qualification

Willingness to undertake a Masters

Degree in Advanced Clinical Practice

Evidence of five years' experience

at a senior level (Band 6 or above/
