Advanced Clinical Practitioner

We have an exciting opportunity for qualified Advanced care practitioners to join our team within Cardiology Services at SWBH.
The roles will involve a presence at the 'front door', assessing, diagnosing and treating patients presenting with chest pain and /or cardiac conditions so we can facilitate earlier admission, signpost to our services or discharge if appropriate.
ACPs will also work autonomously where appropriate, you will provide an early review of patients in the acute setting and emergency department. There will be opportunities to take over running Rapid Access Chest Pain clinics in our outpatient setting. As this is a newish service to SWBH, we are open to building on your previous skills and experiences to support and improve our service.
We are looking to expand our current team of 2 trainee ACP, experience in only cardiology is preferable but not essential
First point of contact for patients presenting in ED with cardiac sounding chest pain. Review of patients that have been referred to AMU or medical SDEC out of hours.
Undertake comprehensive clinical assessment and history of patients' condition, requesting and interpreting diagnostic test results,
Signposting to other services we offer including enhancing our Rapid Access Chest Pain clinics
Reviewing, admitting and discharging patients where appropriate
Providing professional leadership and support within a multi disciplinary team
Developing services that will see patient benefits within our cardiology service.

The Cardiology service is a busy and proactive team, supported by 16 Consultants, with individual abilities that form the team we are, from front door primary care through to rehab, we offer a large cover of this specialism. We have cross site diagnostic department, acute heart failure Cardiac rehab team in our patch as well as two 32 inpatient beds, a cardiac Day case unit and 2 Cardiology Catheter labs.
The department is looking to develop our current service, both as part of our planned move to MMUH, where we will be together in 1 acute area, but also to ensure we have continued capacity for the increasing number of Cardiac patients in the local area we serve.
The ACP roles are part of our development and we believe will greatly enhance the care of our patients.
