**Post**: Phlebotomist Accountable to: Practice Manager and GP Partners Place of work: Elm Lane and Chapeltown Surgery Job Purpose Working as part of a practice multidisciplinary team, delivering care within their scope of practice to our patient population, across both sites. Carrying out blood tests and blood pressure monitoring. Key Responsibilities: The following are the core responsibilities of the phlebotomist/Support Worker. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a.

Carry out blood pressure and pulse rate, recording findings accurately. b. Facilitate routine BP monitoring, at home, advising patients accordingly c. Carry out BMI checks as directed d.

Act as a chaperone as required e. Undertake venepuncture f. Ensure specimens are recorded and ready for onward transportation g. Help ensure all clinical rooms are adequately stocked and prepared for each session h.

Deliver opportunistic health promotion where appropriate Generic Responsibilities: Record Keeping Document the assessment of need, delivery of care and the outcome of care, ensuring that: - An appropriate entry is made in the patient medical records Appropriate forms are completed as necessary General Treatment Room Duties: Ensure that Infection Control procedures are adhered to. Ensure the Treatment rooms are kept clean and tidy. Ensure all medical equipment is properly cared for and any faults reported as soon as possible. Information Technology Input of clinical data obtained during a consultation, BP etc.

Retrieval of relevant medical information from the patient database for the purpose of medical audit or practice research. Education and Training Prepare for and take part in an annual appraisal. Attend practice meetings, and appropriate training courses as agreed by the GPs. Confidentiality: In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.

They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data Ensure log off procedures are followed and computer passwords and smartcard codes are not shared amongst other team members. All employees are required to: Abide by the Health and Safety at Work Act.

Work with the rest of the team in a polite and respectful manner. Work within the surgery no smoking policy. Work within the surgery policies and procedures.
