**About the Role**:
There is an exciting opportunity for an experienced project manager to join Southwest Central and SS9 PCNs, in the Leigh-on-Sea area, to lead us as we transform the way in which we support our patients, the way our staff are able to work and the sustainability of our general practice services, through the adoption or optimisation of new technology and other initiatives. We are working collaboratively on the engagement and support of this new, exciting role.

The responsibilities of the post-holder will include:

- Overseeing the adoption and/or optimisation of new technology to enhance patient access and experience, and to increase PCN productivity;
- Building relationships and facilitating collaboration between practices and the wider system, to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access)
- Leading an improvement approach to change, including building capability for quality improvement within the PCN, and system-wide approaches to problem solving;
- Supporting the review and improvement of the PCN’s digital maturity;
- Using data, and improving data quality, to:

- understand demand, capacity and activity, and to drive improvements in:

- patient experience of access,
- operational efficiency including better matching capacity to need,
- staff experience at work;
- support the health management of our local population;
- support understanding of the type and intensity of support and training needs of the PCN, and coordinate this support, including through OD programmes;
- facilitate clinically-led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level, to improve access to services for patients.
- Educated to degree level, or equivalent professional experience;
- Experience in digital/IM&T project management delivery;
- Good understanding of the digital agenda within the NHS and social care/care sector;
- Good understanding of current healthcare policy, relating to primary care and integrated care;
- Ability to work on own initiative and organise workload, allocating work as necessary;
- Used to working in a busy environment;
- Adaptability, flexibility and ability to cope with uncertainty and change;
- Ability to deliver a vision and a willingness and drive to adapt to NHS targets and business planning, whilst keeping abreast of legislative and contractual changes;
- Self-motivation and forward-thinking approach;
- Excellent organisational, management and planning skills, IT skills and communication skills.
