**Job Summary**:
We are seeking a dedicated and compassionate Mental Health Practitioner to join our team. As a Mental Health Practitioner, you will play a vital role in providing support and care to individuals with mental health conditions. Your primary responsibility will be to assist patients in their daily activities and ensure their well-being. This is an excellent opportunity for individuals interested in making a positive impact on the lives of others. You will work closely with three GP Practices within the PCN.
**Qualifications required**
- Diploma/Degree or any other post graduate qualification or equivalent experience.
- Qualification in Mental Health Nursing, or Occupational Therapy or Social Work.
- Professional registration
**Experience required**
- Completion of mentorship / preceptorship course or in the process of completion.
- Significant post registration experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness.
- Experience of coaching (supervision and appraisal of other junior clinicians).
- Demonstrable expertise in complex assessment of people with mental health issues
- Experience of leading clinical practice.
- Experience of providing range of evidence-based interventions to people with a variety of mental health problems and serious illness.
- Suitable for someone whois proactive and passionate about innovation and providing a high-quality holistic approach to the care of people who have been identified as in need of mental health support in a primary care setting.
**Key Responsibilities**
- To support the adoption of the GPimhs model within the PCN.
- To provide a lead role in supporting the PCN in triaging mental health concerns raised by local agreed pathways at local PCN level providing highly specialist advice and taking responsibility for the development and implementation of specialist mental health training to the PCN.
- To provide a navigation role to help GPS make appropriate referrals to the relevant community and secondary care services - bridging the gap between primary care services such as IAPT and secondary care, working in partnership across the system.
- To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialized advice concerning care when appropriate and liaising with GPs.
- To act as the first point of contact for any clinical concerns raised by any GP’s and ARRs MHP, to offer advice, support and sign-posting to appropriate agencies.
- To receive and communicate sensitive information to clients in situations which may be highly emotive, and where barriers to acceptance and understanding may need to be overcome, requiring developed communication and empathic skills.
- To maintain and update patient records on the computer in accordance with practice policies and professional guidelines e.g. NICE and other evidence-based guidelines.
- To liaise with appropriate resources when necessary (e.g. Welfare Rights, Housing Agencies, Voluntary Agencies, Day Services etc.).
- To participate in professional meetings as necessary where there is notification of serious concern i.e. Safeguarding and risk management concerns or in multi-disciplinary reviews.
**Job Types**: Full-time, Fixed term contract
Contract length: 12 months
**Salary**: £24,540.00-£30,201.00 per year
