We are looking for an enthusiastic and highly motivated Community Mental Health Practitioner to join our Community Mental Health Team at the Greenfields Centre in Tunstall, Stoke-on-Trent.

This newly developed post will be based, within a multi-disciplinary Community Mental Health Team, working closely with a number of professional groups.

Through the community transformation project, we are developing innovative pathways to revolutionise the way community services are delivered and looking for a mental health practitioner to support with the group element of the pathways.

Your primary role will be to work with the whole MDT to support the development and role out of groups linked to specific pathways.

As part of an Outstanding Trust (CQC) you would be working in a highly-motivated community multi-disciplinary team. There will be opportunities and support for continuing professional development and to actively participate in quality innovation, research and audit. There will also be opportunities to lead on particular projects for the team.
