Palliative Care Activities and Family Support Worker

Scope of Job To offer, coordinate and provide memory making activities or mementoes for babies, children, young people and their families who are receiving palliative and end of life care in the most appropriate setting home, hospice or hospital. To act as a creative resource for the multidisciplinary team when planning and coordinating bucket list for babies, children, young people and their families during palliative and end of life care. To offer practical, social and therapeutic support for the wider family during the end-of-life phase. To be a key source of social and emotional support for siblings before, during and after end-of-life care.
To facilitate and forge external links with community services and providers. To contribute to the development of the rapid response and sibling support services. To work in partnership with the Rapid response and family support leadership teams to monitor and evaluate the impact of memory making and sibling support. Dimensions and Limits of Authority The post holder will hold delegated line management responsibility for their volunteer team following due process.
Appropriate advice should be sought from the Rapid response and family support leadership teams. The post holder will be expected to coordinate activities working within a set budget seeking appropriate sign-off from a budget holder before purchases are made. To work collaboratively with other multi-disciplinary professionals from health, education, and social care to provide joined up and seamless care and support. To work without supervision as an autonomous practitioner referring and working in partnership with the wider multidisciplinary team.
Proven experience of providing activities or support for babies, children, young people and families. Excellent interpersonal and communication skills Minimum functional skills level 2 in English & Maths Proven experience of working with children and young people with complex health needs Experience of working in a clinical environment Experience of coordinating activities/events. PART 2: MAIN DUTIES AND RESPONSIBILITIES The Rapid response and sibling support service operates a 7-day service and as such, this role requires working flexibly as required. To coordinate, organise and provide all memory activities for babies, children and young people who are receiving palliative or end of life care in a setting of familys choice hospice, hospital or home.
To ensure all activities are responsive to a babys/childs/young persons individual developmental, social, spiritual and cultural needs. To attend the daily rapid response handover and monthly case load review so memory making is integral to the care provided and that the post holder is up to date with the babies/childs or young persons clinical status. To offer practical and emotional support to the wider family in particular siblings before, during and following end of life care in a setting of the familys choice. To work in partnership with family support team to share the delegated responsibility for the planning, coordination and implementation of sibling therapeutic and social activities which will involve some overnight residential trips.
To undertake risk assessments for any activities and events to ensure they are safe and fit for purpose. To liaise with the fundraising, communications teams, and the administration team in relation to all external requests for information, external visits and activities that will involve children and young people. To have knowledge of the specialist equipment used with memory making for CYP and can provide instruction/teaching on the use of such with internal and external teams. To have a knowledge of external wish granting agents and be a key for making such referrals working with the familys key worker.
To take responsibility for maintaining positive relationships with external memory making service providers such as JUMP, potting shed, bump photography, perfect charms etc. ensuring all staff know any related procedures or referral flows. To share responsibility within the Hospice to maintain a safe but homely environment taking account of health and safety, including infection control and risk assessments. To be aware and working to Safeguarding and Prevention of Children and Vulnerable Young Adults policies and procedures To comply with moving and handling policies and procedures To participate in hospice special interest groups To be able to undertake the mental, physical and emotional demands of the role, whilst at the same time taking care to safeguard their own health and safety as well as fellow care team members, children, young people and their family.
