Spiritual Care Coordinator

Job Summary The post holder will provide spiritual and religious care and support to; All patients and their families, or support networks, regardless of faith or life stance. Hospice staff and volunteers in relation to their roles at the hospice. Co-ordinate and lead a team of spiritual care volunteers, within the hospice and to work with other external Faith Leaders to ensure delivery of seamless service to meet the needs of all users. Work as a member of the Patient and Family Support Team, supporting hospice services to provide high quality, patient centred, palliative care.
Key Responsibilities 1. To identify and support the spiritual, psychological and cultural needs of hospice patients and those close to them. 2. To help staff to identify the spiritual need and distress being experienced by patients and those close to them.
3. To support patients and those close to them in their distress and access external and internal multi-disciplinary support as required. 4. To support patients and those close to them in such a way that their faith needs are met.
5. To liaise with visiting faith leaders, ensuring good communication as required (respecting confidentially boundaries). 6. To ensure that spiritual needs and concerns, and requirements of faith tradition are documented appropriately on the patient records.
7. To ensure that staff have sufficient information and are supported to meet these needs. 8. To respect the wishes of patients and/or those close to them to decline spiritual or chaplaincy support.
9. To participate in multi-disciplinary and case meetings in agreement with Patient and Family Support Services Manager. 10. To discuss requests to officiate at funerals with Patient and Family Support Services Manager, and follow agreed policies.
11. To work with the bereavement team in the delivery of regular memorial services and other events. 12. To work out of regular hours on occasions that this is required.
13. To be responsible for the hospice Quiet Space, ensuring this is a space which supports reflection for those of any faith or none. 14. Organise and participate in remembering events including services, for patients, families, carers and staff.
15. To continually look at how to develop the spiritual care service to ensure it is meeting the needs of the patients, those close to them and the hospice community. 16. To be able to work cooperatively within the hospice team.
Management 1. Participate in relevant groups and meeting forums identified by PFSS managers, supporting improvement in the care of, and services for, patients and those close to them. 2. To identify need and gaps in services and help identify developments to meet these needs.
3. To lead and manage the spiritual care / chaplaincy volunteers with support from Volunteer Services Manager and Patient & Family Support Services managers as required, including induction and regular supervision meetings. 4. Be responsible for compliance with own professional body requirements in terms of code of conduct, professional practice, expertise and knowledge, including maintaining continuous professional development.
Organisational Policy and Procedures 1. Maintain knowledge of and comply with the relevant clinical Hospices policies and procedures. Research and Audit Participate in clinical audit where indicated. Respond to identified needs, developing and modifying practice where required.
Quality of Service Provision of a safe environment where patients can freely express themselves without fear of judgement or condemnation. Ensure the Chaplaincy service is responsive and available to all patients, relatives and carers by a Chaplain, or a member of the team, being available to the Hospice at key times. To contribute to the provision of high-quality religious, spiritual, and pastoral care for all patients, those close to them and the staff at the Hospice. Education To support the induction of staff and volunteers on spiritual care.
To be involved in educational programmes arranged for employees, volunteers, and other external participants, as appropriate. To take part in the provision of opportunities for in-service training to enable team members to develop their understanding of Spiritual Care across a multi faith/no faith arena. To help publicise the Spiritual care available to ensure that patients, families, and carers are aware of support. To represent the Hospice within the wider community, assisting in developing services that meet the needs of all stakeholders.
This will include meeting with local Church leaders and leaders of other faith/cultural communities.
